During the Covid-19 Crisis, you cannot enter The Surgery. Our phone lines will open from Monday to Friday at 9:00 am until 16:45 pm each day. We are offering phone consultation with a Doctor. All telephone calls will be triaged by a Receptionist first.

Document collections, prescription collections are available for collection until 17:00.

Doctor appointments are allocated ten minutes. However very sick patients require a longer time.  We appreciate your patience if you are delayed.

If you are looking for an appointment for our special services e.g. Long acting Contraception, Menopause, Joint injections or minor surgery please let the Receptionist know.

Please be aware you need to make a double appointment for special services.

Phone NEDOC 1850 777 911. When our surgery is closed you will be seen at NEDOC.

In an emergency please call 999 for an ambulance or go to your nearest Hospital Emergency Department.

If you look at our website ‘thesurgerydunshaughlin.ie’ you can download our prescription order form.  Please fill out all your medication, the dose and how often you take them.  Please complete with as much information as possible as this will help speed up our prescription process.  If you would rather drop in a prescription form you can collect a hardcopy from our front desk and return it to us. Alternatively you can request that your pharmacy order a repeat prescription on your behalf if neither of the above options are feasible.  It usually takes 48 hours for prescriptions to be ready.

Please drop in Hospital prescriptions on your way home from the hospital so that we can have them ready for you as soon as possible.  The pharmacy will give you a five day supply in the meantime.

When you have seen and been examined by the Doctor he or she will decide the appropriate tests that need to be taken and will send you to one of our nurses for the appropriate tests.

Please register with cervicalcheck.ie, once your letter has been posted out to you you can make an appointment for your test.

Please bring your letter with you.

All test results are reviewed by the Doctor. All patients should ring the surgery one week after their tests to receive the results.

If they are normal or stable you will receive the results from the receptionist. If they are not normal you will be notified by either one of our nurses or the Doctor.

You must be first seen by the Doctor to certify you as unfit for work.  If you have recently seen the Doctor we may be able to write or extend a Certificate.

Please use the links below for the HSE and Department of Social Welfare.  We do not provide these forms but we do fill in the medical sections. If you are collecting forms please be aware that it can take up to a week for some forms to be completed by the Doctor as sick patients take priority.  Completed forms or documents can be collected daily up to 5.00pm.

We are sending all “Certificates of Incapacity for Work” directly to the Department of Social Protection. You should order an IB1 form from your local welfare office and send it back to them filled out. You can order an IB1 form by calling 1890 800 024 or by email at [email protected]. If you are claiming illness benefit due to Covid-19 you can fill in your IB1 form online using the services.mywelfare link. Please note you can only send the IB1 form online if your claim is Covid-19 related, otherwise you will have to provide a hard copy.
